To confirm your coverage and understand your benefits, please contact your insurance company and our office prior to your appointment. Please keep in mind that you are responsible for any costs that are not covered by insurance, which includes your co-pays and out-of-pocket expenses. Payment is due at the time of your appointment and our office accepts:
